Joining our vision program is fast and easy. Completion of our simple form below will enroll you as a Corporate Member of Vision Discount Plan.

All member discounts will be available to your employees and their dependent family members to use. You only need to enroll for the company. - Your Privacy is Important To Us

We will not sell your information to anyone or any company. We do need your contact information and a valid e-mail address to send your membership cards and to provide you with updated information or occasional "members only" opportunities.
How To Begin Saving
Vision Discount Plan requires no waiting.
- Print Your Temporary Voucher
After completing the Enrollment Form and clicking "Join," you will automatically receive a Temporary Voucher for printing. Please note that each Temporary Voucher will provide your employee and their dependent family members their member discounts until you receive your permanent membership cards. So, there's no waiting if your employees need an eye exam or other related products. We encourage you to print or photocopy as many Temporary Vouchers as you need for all of your employees. - Bring Voucher or Membership Card to All Appointments
Please remind your employees to bring the Temporary Voucher with them for an exam or other benefit related visit; it is required and works just like the membership card. Please distribute the permanent membership cards to your employees as soon as they arrive. - Request Appointments Online
Finally, members will also have the option to request an appointment for eye exams. With a few clicks of the mouse you and your employees can schedule your next eye exam in just a few minutes!
