How To Enroll

Joining our vision program is fast and easy. Completing our simple form below will enroll you as an Individual Member of Vision Discount Plan.

• Just 1 Enrollment Per Family Required
  All of your member discounts will also be available to your dependent family members so you only need to enroll for yourself. Just be sure you have your card with you if you are with a dependent family member or spouse.
• Your Privacy is Important To Us
  At Vision Discount Plan, we value our relationship with you. We will not share your personal information with any third party or outside vendor. We will not sell your information to anyone or any company. Please take a few moments to review the important information on the Privacy Policy page of this website.
• Please Complete ALL Fields in the Enrollment Form
  We do need your complete contact information and a valid e-mail address to send your membership card and to provide you with updated information or occasional "members only" opportunities.

How To Begin Saving

Vision Discount Plan requires no waiting.

• Print Your Temporary Voucher
  After completing the Enrollment Form and clicking "Join," you will automatically receive a Temporary Voucher for printing. This Temporary Voucher will let you enjoy your member benefits until you receive your permanent membership card. So, there's no waiting if you need your eye exam or other related products.
• Bring Your Voucher or Membership Card to All Appointments
  Please remember to bring the Temporary Voucher with you for your exam; it is required to receive your member benefits until you receive your permanent membership card.
• Request Appointments Online
  Finally, you will also have the option to request an appointment for your eye exam. With a few clicks of the mouse both you and your whole family can schedule your next eye exam in just a few minutes!
